FHIR Artifacts

Page standards status: Informative

This page provides a list of the FHIR artifacts defined as part of this implementation guide.

Behavior: Capability Statements

The following artifacts define the specific capabilities that different types of systems are expected to have in order to comply with this implementation guide. Systems conforming to this implementation guide are expected to declare conformance to one or more of the following capability statements.

TOC CapabilityStatement

This Section describes the expected capabilities of the PACIO Transitions of Care (TOC) Server actor which is responsible for providing responses to the queries submitted by the TOC Requestors.

The TOC information is conveyed via a Bundle.

Structures: Resource Profiles

These define constraints on FHIR resources for systems conforming to this implementation guide.

Transition of Care Bundle

This is a profile for a Transition of Care Bundle, which includes the necessary resources for documenting a patient's transition from one care setting to another.

Transition of Care Composition

This profile defines constraints that represent common administrative and demographic concepts for transitions of care information used in US Realm clinical documents.

Transition of Care Document Reference

This profile defines constraints that represent the information needed for a Transition of Care document on a FHIR server.

Structures: Extension Definitions

These define constraints on FHIR data types for systems conforming to this implementation guide.

Point of Contact

The Point of Contact Extension represents the individual or organization that can be contacted for information about the patient or their care.

Terminology: Value Sets

These define sets of codes used by systems conforming to this implementation guide.

Transitions of Care DEL LOINC Terms Value Set

This value set includes those LOINC terms that are used by the DEL.

Terminology: Code Systems

These define new code systems used by systems conforming to this implementation guide.

Transition of Care Temporary Code System

Codes to be used as placeholders until they have a formal assigned code from a recognized terminology steward.

Example: Example Instances

These are example instances that show what data produced and consumed by systems conforming with this implementation guide might look like.
