This profile defines constraints that represent common administrative and demographic concepts for transitions of care information used in US Realm clinical documents.

Representation Guidance

  • Assumes that all of the sections were assembled for sending at same time, identified by the Composition.date element.
  • TO DO: clarify where we find the timestamps for each of the included sections…
