The US Core MedicationAdministration Profile inherits from the FHIR MedicationAdministration resource; refer to it for scope and usage definitions. This profile meets the requirements of the U.S. Core Data for Interoperability (USCDI) Medication Administration Data Element of the Medications Data Class. It sets minimum expectations for the MedicationAdministration resource to record, search, and fetch information about the event of a patient consuming or otherwise being given a medication. It specifies which core elements, extensions, vocabularies, and value sets SHALL be present in the resource and constrains how the elements are used. Providing the floor for standards development for specific use cases promotes interoperability and adoption.

This artifact is new for US Core Version 10.0.0-ballot

Example Usage Scenarios:

The following are example usage scenarios for the US Core MedicationAdministration Profile:

  • Query for MedicationAdministration resources belonging to a Patient
  • Query for a Patient's MedicationAdministration resources by status or administration time
  • Record or update a Patient MedicationAdministration

Mandatory and Must Support Data Elements

The following data elements must always be present (Mandatory definition) or must be supported if the data is present in the sending system (Must Support definition). They are presented below in a simple human-readable explanation. Profile specific guidance and examples are provided as well. The Formal Views below provides the formal summary, definitions, and terminology requirements.

Each MedicationAdministration Must Have:

  1. a status (e.g., completed, in progress, not done)
  2. a medication1
  3. a patient
  4. when the medication was administered4

Each MedicationAdministration Must Support:

  1. a reason the administration was not performed3
  2. who administered the medication2
  3. the authorizing prescription
  4. free text dosage instructions (the SIG)
  5. the route of administration
  6. the dose

1,2,3,4 see guidance below

Profile Specific Implementation Guidance

This section provides detailed implementation guidance for the US Core Profile to support implementation and certification.

  • The MedicationAdministration resource represents the USCDI Medication Administration data element. The Medication List guidance remains based on MedicationRequest; a MedicationAdministration query is not a substitute for it.
  • A status code of "not-done" is used when a medication was not administered.
    • 3MedicationAdministration.statusReason communicates why the medication was not administered, such as patient refusal. Note that this binding is provisional, pending the review of the terminology aligned with the HL7 FHIR US Quality Core Implementation Guide.
  • 4MedicationAdministration.effective[x] is mandatory and records when the administration took place, or when it was to have taken place when MedicationAdministration.status is "not-done". Use effectiveDateTime for an essentially instantaneous administration, such as swallowing a tablet or giving an injection, and effectivePeriod for an administration with a duration, such as an infusion. An effectivePeriod with no end time records an administration that is still under way.
  • 1 This Profile can represent a medication using a code or reference to a Medication resource.
    • The Server systems are not required to support both a code and a reference, but SHALL support at least one of these methods.§
    • The Client application SHALL support all methods.§,§
    • When using a code, RXNorm concepts are used. They are defined as an extensible binding to .medicationCodeableConcept. USCDI recommends the National Drug Codes (NDC) as an optional terminology. They MAY be supplied as an additional coding element.§
      • Systems that primarily rely on NDC codes instead of RxNorm to represent medications can use the National Library of Medicine's (NLM) NDC to RxNorm mappings to aid in additional codings.
    • When referencing a Medication resource in .medicationReference, the resource may be contained or an external resource.§,§ If an external reference to a Medication resource is used, the Server SHALL support the _include parameter for searching this element.§
  • 2The profile element MedicationAdministration.performer.actor communicates the individual level provenance author data corresponding to the U.S. Core Data for Interoperability (USCDI) Provenance Author Data Elements.

    Notes:

    Quick Start

Below is an overview of the required Server RESTful FHIR interactions for this profile - for example, search and read operations - when supporting the US Core interactions to access this profile's information (Profile Support + Interaction Support). Note that systems that support only US Core Profiles (Profile Only Support) are not required to support these interactions. See the US Core Server CapabilityStatement for a complete list of supported RESTful interactions for this IG.

    • See the Scopes Format section for a description of the SMART scopes syntax.
    • See the Search Syntax section for a description of the US Core search syntax.
    • See the General Requirements section for additional rules and expectations when a Server requires status parameters.
    • See the General Guidance section for additional guidance on searching for multiple patients.

    US Core Scopes

Servers providing access to administered medication data SHALL support these US Core SMART Scopes:

    Mandatory Search Parameters:

The following search parameters and search parameter combinations SHALL be supported:

    1. SHALL support both read MedicationAdministration by id AND MedicationAdministration search using the _id search parameter

      GET [base]/MedicationAdministration?_id=[id]' or 'GET [base]/MedicationAdministration/[id]

      Examples:

      1. GET [base]/MedicationAdministration/12354

      2. GET [base]/MedicationAdministration?_id=12354

Implementation Notes: Fetches a single US Core MedicationAdministration Profile or a search Bundle containing a US Core MedicationAdministration Profile resource matching the id (see Parameters for all resources).

    2. SHALL support searching for all medication administrations for a patient. The server application represents the medication using either an inline code or a contained or external reference to the Medication resource. using the patient search parameter

      • Including support for these _include parameters: MedicationAdministration:medication

      GET [base]/MedicationAdministration?patient={Patient/}[id]

      Examples:

      1. GET [base]/MedicationAdministration?patient=Patient/1137192

      2. GET [base]/MedicationAdministration?patient=Patient/1137192&_include=MedicationAdministration:medication

Implementation Notes: Fetches a bundle of all US Core MedicationAdministration Profile resources for the specified patient ( see how to search by reference).

    3. SHALL support searching for all medication administrations for a patient for a given status (for example, all completed administrations or all administrations that were not performed) using the combination of the patient and status search parameters

      • Including support for these _include parameters: MedicationAdministration:medication

      • Including support OR search on status (e.g.status={system|}[code],{system|}[code],...)

      GET [base]/MedicationAdministration?patient={Patient/}[id]&status={system|}[search_code]{,{system|}[code],...}

      Examples:

      1. GET [base]/MedicationAdministration?patient=Patient/1137192&status=completed

      2. GET [base]/MedicationAdministration?patient=Patient/1137192&status=completed&_include=MedicationAdministration:medication

Implementation Notes: Fetches a bundle of all US Core MedicationAdministration Profile resources for the specified patient and status ( see how to search by reference and how to search by token).

    Optional Search Parameters:

The following search parameters and search parameter combinations SHOULD be supported

    1. SHOULD support searching for all medication administrations for a patient by the time the administration took place using the combination of the patient and effective-time search parameters

      • Including support for these _include parameters: MedicationAdministration:medication
      • Including optional support for AND search on effective-time (e.g.effective-time=[date]&effective-time=[date]&...)

      • Including support for these effective-time comparators: "gt", "lt", "ge", "le"

      GET [base]/MedicationAdministration?patient={Patient/}[id]&effective-time={gt|lt|ge|le}[dateTime]{&date={gt|lt|ge|le}[dateTime]&...}

      Examples:

      1. GET [base]/MedicationAdministration?patient=Patient/1137192&effective-time=ge2026-01-01T00:00:00Z

      2. GET [base]/MedicationAdministration?patient=Patient/1137192&effective-time=ge2026-01-01T00:00:00Z&_include=MedicationAdministration:medication

Implementation Notes: Fetches a bundle of all US Core MedicationAdministration Profile resources for the specified patient and administration time ( see how to search by reference and how to search by date).
