ICP Case Create is to be used by the supplier to furnish ACC with the required information following the completion of the triage assessment and the decision to accept the patient into their ICP service.

This profile supports suppliers needing to inform ACC of a patient being exited from their ICP service.

This profile supports suppliers needing to inform ACC of a change to the selected Service Bundle and/or Exceptional Funding Required.

ACC ICP Patient

The ACC ICP Patient resource based on NzPatient. Used to verify that an ICP Case has a valid claim.

ACC ICP Referral Decline Resource

The ICP Referral Decline profile is to be used by ICP suppliers to furnish ACC with the required information following them declining an ICP referral.

ACC Integrated Care Pathway Clinical Observation

This resource profile supports suppliers providing site-specific clinical measures to ACC for ICP purposes.

When the valueQuantity is provided, the value is expressed as a percentage from the following calculation:

ACC Integrated Care Pathway Site-Specific PROM

This resource profile supports suppliers providing a site-specific patient reported outcome measure (PROM) to ACC.

These PROMS must be submitted through the PROM and Clinical measures API via the supplier's PMS at these time-points:

  • Baseline (at entry to the pathway following triage assessment)
  • Mid-point
  • Exit
  • Ad-hoc measures may be taken at any time (for example, at the beginning of using exceptional funding if this has been required).
