eLTSS Profiles

These are the Resource Profiles as designed by the eLTSS project.

The eLTSS Implementation Guide follows the Must Support requirements as defined in US Core.

The eLTSS IG contains eLTSS Profiles to show the mapping of the eLTSS Dataset to FHIR elements. Many Profiles will only contain this mapping in the "Description & Constraints" column of the Differential Table. eLTSS Profiles that are derived from US Core Profiles will only contain additional constraints in the Differential Table that support eLTSS requirements. The US Core constraints will be present in the Snapshot Table.

Profile Description
eLTSS CarePlan Profile Base profile that defines the CarePlan constraints for eLTSS.
eLTSS Claim Profile Defines the Claim constraints for eLTSS.
eLTSS Condition Profile Defines the Condition constraints for eLTSS. Not all SNOMED codes for potential conditions (assessed needs) are included in the value set. For additional code requirements, please submit a request to the eLTSS Confluence page
eLTSS Contract Profile Defines the Contract constraints for eLTSS.
eLTSS EpisodeOfCare Profile Defines the EpisodeOfCare constraints for eLTSS.
eLTSS Goal Profile Defines the Goal constraints for eLTSS.
eLTSS Location Profile Defines the Location constraints for eLTSS. This profile should only be used for Service Delivery Address when the service or support is provided outside the person's home address. Use Person.address when service or support is provided in the person's residence.
eLTSS Observation Profile Defines the Observation constraints for eLTSS.
eLTSS Patient Profile Defines the Patient/Person constraints for eLTSS.
eLTSS RelatedPerson Profile

This profile sets minimum expectations for the RelatedPerson resource to record, search, and fetch basic administrative information about persons with a legal or familial relationship to a patient. It is patterned off of USCORE.
