Background

In the field of medical imaging, the Digital Imaging and Communications in Medicine (DICOM) protocol and format are standardly used for data archiving and exchange. This defines, for example, individual imaging modalities and is used for data archiving in the Picture Archiving and Communication System (PACS).

DICOM is linked to the SNOMED-CT clinical terminology system. Other standards and nomenclatures are shared by imaging methods with other patient documentation documents: MKN-10 (national representation of international ICD-10), HL7, LOINC and the like.

The paper template for the electronic request form is the paper request form for the Examination/Treatment Voucher type Z. The specification of the content of the paper request form is given in příloha č. 1 v části 2 k vyhlášce č. 444/2024 Sb., o zdravotnické dokumentaci, as of February 19, 2025, the wording is as follows: A. The request for additional health services contains a) the requested health services, b) justification for the request, c) urgency of the request, d) data on the most recently determined health condition, including those results of laboratory and other auxiliary examinations that are essential for the provision of the requested health services, e) data on the patient's previous treatment and response to it, f) working diagnosis, g) information on essential anamnestic data related to the requested health services.
