中醫門診單病史(ConditionMedicalHistory TWCM)之建議用詞，合併月經史、產科史及男性相關(問)之臨床表現詞彙，以及慢性疾病值集(ICD-10-CM)。一般過去病史及現病史可使用慢性疾病值集、其他代碼系統或僅填文字。
