This section contains concise, well specified, codeable, summary of problems. Problems are ordered by importance (main problems first) during hospital stay. Description of the problem might be completed with additional details in the medical history section and/or in the Synthesis section.

The diagnostic summary contains a concise, well specified and codeable summary of problems as they were recognised at the end of hospitalisation, after all examinations, ordered by importance with the main problems first. In accordance with international recommendations it differentiates between problems treated during the hospital stay, meaning problems that were the subject of diagnostics, therapy, nursing or continuous monitoring, and other untreated problems, which are recorded only where they matter for continuity of care. Each problem is further classified as present on admission, acquired during the hospital stay, or not classifiable as either. Additional detail on a problem may be provided in the patient history section or in the synthesis section.
