This page provides a list of the FHIR artifacts defined as part of this implementation guide.
These define constraints on FHIR resources for systems conforming to this implementation guide.
| AUDIT-C Total Score |
AUDIT-C total score (0–12) as a quantitative Observation; linked to QuestionnaireResponse if necessary. |

Observation of a patient's current alcohol consumption status. Optionally includes an AUDIT-C score component when alcohol intake is present. |
| Alcohol Use History |
This profile constrains the Observation resource to represent a simple yes/no alcohol use history item. |

Observation indicating whether the patient has had Diabetes Mellitus for 10 years or more. |
| Family History of Cardiovascular Disease |
Observation indicating whether the patient has a family history of cardiovascular disease. |
| Family History of Diabetes |
Observation indicating whether the patient has a family history of diabetes mellitus. |
| Family Member History for screening program |
Family Member History record. |
| Medication Statement for screening programs |
A simplified profile for MedicationStatement focusing on essential elements. |
| Metabolic Syndrome (NCEP ATP III) |
Observation indicating presence of metabolic syndrome according to NCEP ATP III criteria (≥3 of 5 signs). |
| Number of Steps |
The measurement tracks the count of steps taken by an individual during one day. |
| Nutrition |
Observation profile representing the patient's nutrition status (balanced vs. unbalanced). |
| Physical Activity |
This profile constrains the Observation resource to represent the patient's level of physical activity compared to recommended standards. |
| Physical Activity Measurement Device |
Device used to measure physical activity metrics such as number of steps. |
| Tobacco Smoking Consumption |
This profile constrains the Observation resource to represent Tobacco smoking consumption assessment in a patient summary. |
| Tobacco Smoking Stopped - Number of Years Ago |
Records how long it has been since the patient stopped smoking, as a quantitative time interval (e.g., years). |
| Tobacco Type Used |
Records the type of tobacco product currently or previously used by the patient (e.g., cigarettes, cigars, e-cigarettes). |
| Tobacco Use |
This profile constrains the Observation resource to represent Tobacco use assessment in a patient summary. |
| Tobacco Use Duration - Number in Years |
Amount of time (e.g. years) the patient has used smoking or smokeless tobacco products |
These define constraints on FHIR data types for systems conforming to this implementation guide.
| Medication Timing Extension for LT Lifestyle |
The timing instructions for the medication (e.g., 'Before Meals', 'With Food'). |
| Other Activity Description |
Free text description required when the 'Other' option is selected for physical activity or a similar behavioral intervention. |
| Other Desired Dietary Changes |
Additional information provided about desired dietary changes (text field). |
These define sets of codes used by systems conforming to this implementation guide.
| Alcohol Consumption Status |
Whether the patient currently consumes alcohol, does not consume alcohol, or the status is unknown. |
| Alcohol Use Frequency |
Frequency of alcohol consumption. |
| Family Member History Screening Program Diseases |
Indicates whether the patient has a family member history related to the screening program. |
| Medication taken by patient during treatment |
SNOMED CT concepts representing medication given/used during treatment. |
| Metabolic Syndrome Assessment (NCEP ATP III) |
Assessment of metabolic syndrome presence according to NCEP ATP III criteria. |
| Nutrition Status |
Categorical assessment of whether the patient's nutrition is balanced or unbalanced. |
| Physical Activity Level |
Categorical assessment of whether the patient's physical activity meets, exceeds, or falls below recommended levels. |
| Screening Programme |
Represents the various screening programmes supported in Lithuania. |
| Status of the Family History of Cardiovascular Disease |
Indicates whether the patient has a family history of cardiovascular disease. |
| Status of the Family History of Diabetes |
Indicates whether the patient has a family history of diabetes mellitus. |
| Tobacco Smoking Consumption - Cigarettes per day |
Categorical representation of average cigarettes smoked per day. |
| Tobacco Type User |
Records the type of tobacco product currently or previously used by the person (e.g., cigarettes, cigars, e-cigarettes). |
| Tobacco. Current Smoking Status |
HL7 LT SNOMED value set for smoking status. |
| Treatment Status |
Indicates whether a screening treatment or intervention has been provided for a risk factor. |
| Yes-No options |
Selection Yes or No. |
These are example instances that show what data produced and consumed by systems conforming with this implementation guide might look like.
| Alcohol Use Disorder Identification Test - Consumption [AUDIT-C] |
The Alcohol Use Disorder Identification Test - Consumption (AUDIT-C) is a 3-item alcohol screen that can help identify persons who are hazardous drinkers or have active alcohol use disorders (including alcohol abuse or dependence). It is a subset of the 10-question AUDIT instrument and has been found to be as effective as the full AUDIT in identifying patients who are hazardous drinkers or have active alcohol use disorders.
