HL7 European EHDS Implementation Guide

Introduction and Purpose

This Implementation Guide (IG) provides a European FHIR-based foundation to support the European Health Data Space (EHDS). It brings together selected HL7 Europe implementation assets in a single guide to support consistent, interoperable, and reusable health data exchange across European contexts.

Its purpose is to simplify the adoption of FHIR for EHDS-related use cases by providing a coherent set of conformance resources that can support harmonized implementation, cross-border interoperability, and more consistent use of health data across systems and jurisdictions.

This guide currently includes implementation assets derived from the following HL7 Europe guides:

  • HL7 Europe Base and Core FHIR IG — Foundational profiles and reusable conformance resources for core FHIR implementation in the European context.
  • HL7 Europe Laboratory Report IG — Profiles and related assets for laboratory reporting and diagnostics.
  • HL7 Europe Imaging Report R5 IG — Profiles and related assets for imaging studies, imaging reports, and associated clinical information.

Audience

This guide is intended for implementation guide authors, system vendors, integrators, architects, and other stakeholders involved in designing, implementing, or maintaining FHIR-based data exchange in support of EHDS-related use cases. Readers are expected to have at least a basic familiarity with FHIR and to refer to the core FHIR specification where additional background is needed.

Implementation, testing, and feedback on this guide are encouraged as part of its continued development.

Scope and Key Content

This guide provides a reusable set of European conformance assets intended to support EHDS-aligned implementations, including:

  • Base and Core Profiles: Core FHIR resource profiles and related conformance resources adapted for European healthcare interoperability needs.
  • Laboratory Reporting: Profiles and supporting artifacts for laboratory observations, reports, and diagnostics workflows.
  • Imaging Reporting: Profiles and supporting artifacts for imaging studies, imaging reports, and related clinical data exchange.
  • Terminology Resources: Reusable terminology assets, including ValueSets and other conformance resources needed for consistent data representation.
  • Implementation Foundation: A common European baseline that can support cross-border exchange and serve as a foundation for more specific national or domain-level implementation guides.

These artifacts are intended to support reuse and harmonization across EHDS-related implementations while allowing further specialization where required by specific use cases or jurisdictions.
