Da Vinci

Da Vinci is an HL7-sponsored project that brings together U.S. payers, providers, and technology suppliers to help payers and providers improve clinical, quality, cost, and care management outcomes using FHIR-related technologies. The project organizes meetings (face-to-face and conference calls) and connectathons to find ways to leverage FHIR technologies to support and integrate value-based care (VBC) data exchange across communities. Da Vinci identifies value-based care use cases of interest to its members and to the broader community.

The process that Da Vinci has adopted includes:

  1. identifying business, clinical, technical, and testing requirements
  2. developing and balloting a FHIR-based implementation guide (IG)
  3. developing reference implementations (RIs) to demonstrate that the concepts in the IG are possible to implement
  4. piloting the standard
  5. supporting the production use of the IG to enable exchange of data to support interoperability for value-based care

All Da Vinci work adheres to a common set of guiding principles.

The Da Vinci DTR project

This standard aims to streamline the process of collecting patient documentation required by health insurance payers for purposes like prior authorization and claims processing. It enables payers to express their documentation requirements in a computable format using FHIR Questionnaires, allowing providers to easily gather and submit necessary information.

The standard automates data collection by leveraging Clinical Quality Language (CQL) to extract existing patient data from EHRs, reducing manual entry and errors. It supports both pre-defined questionnaires and adaptive forms that adjust based on user responses.

Key features include the use of FHIR Questionnaires for defining documentation requirements, CQL for data extraction and logic implementation, and SMART on FHIR integration for seamless EHR compatibility.

This approach benefits providers by reducing administrative burden, payers by standardizing documentation requirements, and patients by potentially expediting access to care. It also creates opportunities for EHR vendors and app developers to integrate documentation functionality into their systems.

The standard aligns with US Core Implementation Guides and addresses HIPAA privacy regulations by emphasizing limited data access for specific documentation needs.
