The following questionnaires SHALL be used to gather the necessary data for this implementation guide. The questionnaires are based on SDC Questionnaire resource and expect a us-core QuestionnaireResponse resource is returned. The questionnaires defined in this IG are based on the us-core profiles, and use the us-core profiles as their parent. This allows for maximum reuse of the us-core profiles, while still allowing for the necessary customizations to meet the needs of CoBHRM.
Questionnaire used during admission process to collect demographic, social, clinical, and administrative information from patients entering behavioral health services
Questionnaire used during a behavioral health episode of care to collect demographic and administrative information from patients receiving behavioral health services
Questionnaire used during a behavioral health episode of care to collect severity information about a mental health or substance use diagnosis
Questionnaire used to collect demographic and administrative information from patients being discharged from behavioral health services
Behavioral Health Administration Special Connections baby questionnaire for collecting information about a baby born to a person enrolled in the Special Connections program
Behavioral Health Administration Special Connections Pregnancy/Postpartum Risk Screening questionnaire for collecting information about a pregnant or postpartum person's risk factors
