This Implementation Guide is produced by HL7 Europe, specifically the Project Team for the Hospital Discharge Report, European Patient Summary, and Base Implementation Guide.

It reflects the substantial contribution of a broad and diverse community of 224 individual contributors, representing 29 countries worldwide, including 23 European countries (Austria, Belgium, Czechia, Denmark, Estonia, Finland, France, Germany, Greece, Hungary, Ireland, Italy, Netherlands, Norway, Poland, Portugal, Romania, Slovakia, Slovenia, Spain, Sweden, Switzerland, and the United Kingdom). This collaborative effort also includes representatives from the MyHealth@EU Solution Providers. The development of this Implementation Guide has been supported by the xShare, XpanDH projects and the Xt-EHR Joint Action, which provided expertise, alignment with European health policy priorities, and validation of the specifications to ensure consistency with the requirements of the European Health Data Space (EHDS)
