En graviditetsmappen består af 3 forskellige dokumenttyper:

• Svangerskabsjournal, som typisk indikerer starten af et nyt graviditetsforløb efter konsultation hos egen læge
• Vandrejournal, som kan optages ved hver konsultation, både hos egen læge og hos jordemoder
• Målinger, som optages løbende ved konsultationer
