Why an ordered item was stopped before it was done. Recorded against the item and not the order: an order carries several items, and one test no longer being needed says nothing about the prescription beside it.

no-longer-clinically-needed No longer clinically needed The clinical question the item was raised to answer no longer needs answering.
patient-being-discharged Patient is being discharged The patient is leaving, and the item would not be done before they go.
done-elsewhere-already Done elsewhere already The same work has already been done, here or at another facility, and repeating it would add nothing.
patient-declined Patient declined The patient did not consent to the item being done.
ordered-in-error Ordered in error The item should never have been ordered. Unlike the others this voids the request rather than stopping it, so the record shows it as never having been validly placed.
parent-order-cancelled Parent order was cancelled The item was stopped because the order it belonged to was cancelled, not on a decision about the item itself.
