This example demonstrates one possible representation of a Patient Story within an AU PS document. In this example, the patient's own narrative is represented as unstructured text in the Patient Story section and uncoded conditions are entered by the patient in the Problem List. AU PS does not currently provide guidance on patient-authored documents or on the Patient Story section, including when it should be used and how its content should be represented. Feedback is sought from implementers on the representation of the Patient Story in AU PS.

Health problems as reported by the patient:
I'm 83 and I've been dealing with prostate cancer that's come back and spread to my bones. Recently I ended up in hospital with terrible pain in my back and stomach, along with some other problems, but they got me sorted and I'm back home now.
My wife and I live in a beautiful valley that we've called home for many years. Our garden is my pride and joy - I've put decades of work into it and being able to potter around out there means everything to me. We're active in our local community too, especially with the choral group that puts on the Christmas carols each year. It's a close-knit community and I know most of the staff at the local hospital.
Our kids are spread out across the country, with some a couple of hours away in the city. That's where my GP is too - I've been seeing him for years and really trust him, but I'll be honest, the two-hour drive is getting harder and harder.
The cancer has caused me quite a few problems - my digestion is troublesome, I've had skin issues, and I'm not fighting off infections like I used to. I'm taking supplements to keep my weight up. I've also noticed I'm not quite as sharp as I was, which is frustrating.
What matters most to me is staying in my own home, in my garden, for as long as I possibly can. I know there may come a time when that's not practical, and I'm trying to accept that. My main worry is keeping the pain under control - I don't want to suffer. If it comes to it, I'd rather have quality time than drag things out. This is my home, and I want to make the most of whatever time I have left here.
