Example Basic: ImagingSelection: DICOM key image instance

R4 issue discussed at the September 2026 HL7 WGM: this Basic example uses the cross-version artifact-title and artifact-description extensions to carry the key-image title and description, but the current publisher/validator reports that these extensions are not permitted on Basic. The intended representation and the related cross-version extension definitions has been discussed at the WGM and a fix to the publisher is underway.
