This guide was produced and developed through a collaborative effort of the Centers for Disease Control and Prevention (CDC), Council of State and Territorial Epidemiologist (CSTE), the Association of State and Territorial Health Officials (ASTHO), the Association of Public Health Laboratories (APHL), public health surveillance practitioners, EHR vendors, and the HL7 Public Health (PH) Work Group for an electronic initial case report (eICR). A list of data elements for electronic initial case reports was developed by the CSTE Electronic Initial Case Report Task Force in collaboration with the CDC. Pregnancy status data for public health reporting were identified by a task force from the Department of Health and Human Services, Office of the National Coordinator for Health IT (ONC).
